   Both. You can call on it, it can be useful to meditate on it now and then, get the fire really raging to keep it tuned up... but mostly, it works autopilot.

   For example, I had a friend over for tea recently... he was kind of stressed, his wife is very bipolar and occasionally makes suicide attempts. Just sitting, talking to him my Tummo fire kicked in, burning off the stuff I was getting from him empathically.
   Mindfullness of surrender is still important tho. Conscious consent. Keep surrendering stuff, your DB will choose whether to send it out your head or burn it up.
   My palms are getting warm, just writing about it.
